MEDICAL RECORD REQUESTS
To submit a request for a person's EMS or Medical Records, requests must be submitted to LAFD EMS Records by email. Medical records are highly confidential and may only be released to the patient or an authorized representative in accordance with applicable privacy requirements.
Step 1 – Complete an Authorization Form
Complete an Authorization for Release of Protected Health Information form. The form provides documentation that the patient has authorized the release of their confidential medical information, or that a representative is legally authorized to act on the patient's behalf.
If the patient is unable to sign and an authorized representative is submitting the request, documentation establishing the representative's legal authority must also be provided. Examples may include:
- Marriage Certificate
- Death Certificate
- Birth Certificate for a minor patient
- Medical Power of Attorney or Advance Health Care Directive
Step 2 – Include a Copy of Your Photo ID
Include a clear copy of the signatory's valid, unexpired government-issued photo ID showing their signature. The signature on the identification must match the signature provided on the authorization form. A discrepancy may result in the request being denied or delayed until a valid authorization is provided.
Step 3 – Submit Your Request
Email the completed authorization form and all required supporting documentation to:
lafd.emsrecords@lafdi10.org
Please ensure that all required information and supporting documentation are included with your request to avoid processing delays.
Some records may be withheld or redacted when required by applicable laws, privacy requirements, or other restrictions governing the release of protected health information.
